Short Term Disability Coverage
For shorter duration disabilities, and at no expense to you, eligible employees qualify for salary continuation at 60% of pre-disability earnings. Benefit commencement and duration are based on your disability. Once approved, short term disability benefits begin on the 1st day after an injury or the 8th day of sickness. Based on the type of disability, benefits could be approved to a maximum duration up to 26 weeks. Claims are subject to pre-existing condition exclusions. Work related injuries are not eligible to be paid under the short term disability benefit. Benefit payments are considered taxable income.

Long Term Disability Coverage
If you are eligible, long term disability benefits may be payable after 180 days of disability. Under the plan, your long term disability benefit will be 60% of your monthly earnings, to a maximum of $10,000, less deductible sources of income. Long term disability benefits begin after 180 days regardless of your qualification for the short term disability benefit plan. Benefits may be payable to the age of 65 or earlier if determined appropriate subject to the terms and conditions of the policy. Long term disability payments are not subject to taxation.
Voluntary Long Term Disability Coverage
If you are eligible, you may be able to purchase voluntary long term disability coverage which pays 60% of your earnings to a maximum monthly benefit of $10,000.
Voluntary Term Life
You may purchase additional, supplemental term life coverage in the amounts for you and your dependents below:
- Employee: $50,000 – $200,000, Spouse: $25,000, Child(ren): $10,000
- The amount of spouse Supplemental coverage may never exceed 100% of the Combined Basic and Supplemental Amount of Life Insurance the employee is eligible for.
